EZ Leads LLC — Consumer Health Data Privacy Policy

1. Scope

This policy supplements our Privacy Policy and applies to “consumer health data” as defined by the Washington My Health My Data Act (RCW 19.373), Nevada SB 370, and the Connecticut Data Privacy Act, and to health information generally, that is recorded on the EZ Leads platform. EZ Leads LLC (“EZ Leads”, “we”) provides software to licensed life insurance agents (“agents”). Agents use it to record information you give them when you ask for an insurance quote. Where this policy says “you”, it means the person the information describes.

2. Consumer Health Data We Collect

An agent may record, in a financial inventory for your quote:

  • Date of birth, height, and weight.
  • Tobacco, nicotine, and vaping use.
  • Medical conditions and diagnoses, including mental health conditions.
  • Prescription medications.
  • Hospitalizations, surgeries, and treatment history.
  • Family medical history.
  • Information about existing insurance coverage that reveals a health condition.

We do not collect precise geolocation to infer health status, and we do not collect genetic data. Where an agent turns on call recording, a call with you may be recorded, and where they also turn on call transcripts, transcribed; the recording and its transcript are kept as the agent's record of the call and contain whatever health details were said on it. Recording runs only when the state on your record and the agent's state both allow a call to be recorded with one party's consent; the agent gives that consent, and no notice is played. Recordings and transcripts are never analyzed to identify anyone by voice or used as a biometric identifier. For a call recorded since October 5, 2026, a redacted copy of the recording and a redacted transcript are also made, with Social Security numbers, bank and payment card details, and dates of birth removed; the rest of the conversation, including any other health details, is unchanged. Only the person who was on the call and the owner of the agent's account can play the original recording or read the original transcript; anyone else permitted to review the call gets only the redacted copy.

3. Sources

Consumer health data comes from you: what you tell the agent in a call, meeting, message, or form when asking about coverage. Agents do not obtain it from data brokers, and the platform does not infer it from your other activity.

4. Why We Collect and Use It

Solely to quote and apply for the life insurance coverage you asked about: to estimate eligibility and price with carriers, to complete an application you choose to submit, and to keep the agent's record of that transaction. EZ Leads processes it only to provide the platform to the agent. It is not used for advertising, profiling for any other purpose, or training models.

5. Who It Is Shared With

  • The agent who collected it and any team member or assistant that agent has authorized on the platform.
  • Insurance carriers, when the agent submits your application or requests an underwriting decision, at your direction.
  • Service providers that host and secure the platform (database hosting, application hosting, error monitoring), and the communications provider that carries, records and transcribes calls (Telnyx), each bound to use it only to provide those services.
  • A call-redaction service (AssemblyAI), when a call with you is recorded: after the call ends, the recording, so it can return a copy of the audio and a transcript with Social Security numbers, bank account and routing numbers, payment card details, and dates of birth removed. It transcribes the whole recording, so it hears whatever health details were said on the call. Nothing is sent for a call that is not recorded. Once we have stored the copies, we ask it to delete its transcript.
  • A quoting service (Insurance Toolkits), when the agent opens a quote from your inventory: your age, sex, state, and whether you use tobacco, with a coverage amount — never your name, contact details, or any other health answer.
  • Our AI provider (Anthropic), when the agent uses Suggestions from calls on a recorded, transcribed call: the new lines of the transcript and the inventory's current answers, so it can propose entries the agent accepts or dismisses. It is bound to use them only to return those proposals, and not to train models.
  • A message-classifying service (TypeSafe AI), when you reply to an agent's text: the words of your message, so the platform can tell what the reply means — a wrong number, a request to stop, a question, a time that suits you. Health details are not what it is asked about, but a reply is your own words, so it sees whatever you wrote. It returns a label, never written text, is bound to use it only for that, and does not train models on it. It also reads the bug reports and feature requests agents send us, with email addresses and phone numbers removed first, so we can sort and route them; an agent describing a problem could mention a health detail about a lead. It returns labels and a confidence only, never written text, and does not train models on them.
  • EZ Leads personnel, for support and security, under access controls and audit logging.

Health data is not shared with advertising platforms, and it does not move with a lead if an agent transfers the lead to another agent through the platform — only the information that arrived with the original inquiry does.

6. No Sale; No Advertising Use

We do not sell consumer health data, and we do not permit agents to sell it through the platform. We do not use it for targeted advertising. No valid authorization to sell it has been, or will be, requested from you by EZ Leads.

7. Your Rights and How to Exercise Them

You may:

  • confirm whether consumer health data about you is held, and access it;
  • have it corrected or deleted;
  • withdraw the consent under which it was collected, which stops further collection and use; and
  • obtain a list of the third parties and affiliates it has been shared with.

Contact the agent you dealt with, or email privacy@myezleads.com. We acknowledge requests within ten business days and respond within 45 days, extendable once by 45 days with notice where the law allows. We may verify your identity by confirming the phone number or email the record holds. If we deny a request you may appeal by replying to our response; we answer appeals within 45 days and tell you how to contact your state attorney general if you are not satisfied. You will not be denied service or charged more for exercising these rights.

8. Retention and Deletion

Health data recorded for a quote is kept while the inquiry is active. Where no policy was purchased, the platform is built to clear the recorded health and financial answers after a period of inactivity set in our retention policy; until that period is set, they are kept for as long as the agent keeps your record. Where a policy was purchased, the record is kept with the transaction for the lifetime of the agent's account, and deleted with it. A call recording and its transcript are deleted one year after the call, or sooner if you or the agent ask for them to be removed; their redacted copies are deleted one year after the call too. On a deletion request the data is removed from the platform and from our service providers within the timelines above, and from backups as they expire. Records of your consent to be contacted, and of any opt-out, are retained separately as required by telemarketing law; they contain no health information.
